if your status is "In a Meeting" then all your contact will see that, and if someone disturbed you then this person that disturbed you although he/she sees you are in a meeting, then it's a bigger problem than a Lync status :)
"In a meeting" and "Do Not Disturb" are totally different, it is better in my opinion for your contacts to see that you are "In a meeting" which mean you are currently in a meeting, but when Do not disturb, then you might not be in a meeting or a call, but you just so busy you don't want someone to disturb you.

Lync sets "In a meeting" statusaccording to your Outlook calendar, you are scheduled to be in a meeting. It only displays your current status, no more meaning. If you are having a remote meeting, maybe you need to IM or share documents with other people.
But if you are having a very important meeting and don't want to be disturbed, you could set your status to "Do Not Disturb" manully.

It shouldn't matter what people would think (if you are in a meeting, etc.). Everyone will have their own different opinions on this. Therefore Outlook shouldn't give us only a subset of options that Lync gives us.
The status options available to select from when setting up an Outlook meeting should match the status options available in Lync (the ones you suggest setting manually).
Please make them available when setting up Outlook meetings.
