I opened a ticket with MS PSS for this, essentially this is "working as intended" and will not be fixed per the product group. I've escalated this with MS folks on the Lync sales side, although I expect little outcome.
Issue happens only when:
- User is a delegate and is set for simultaneous ringing \ call forwarding.
- Delegate has a Desk phone connected to his PC via USB and also to the network by the Ethernet.
- The Lync client is set for Default Audio device as headset and a headset is connected to the PC.
- Call comes to manager.
- Ringing happens at 3 locations, manager's Lync client, Delegate's Lync client and Delegate's Desk phone.
- Delegate answers the call on the Lync client, but it goes to Desk phones speaker instead of headset.
Issue doesn't happen even if any one of the following is true:
- User is not a delegate
- Desk phone is connected to only network via Ethernet and USB is not connected.
- Lync Default audio device is not set to headset.
As we discussed, this is a known issue as multiple cases are logged in for the exact same problem and upon in-depth analysis internal bugs were opened to fix this issue. All the bugs raised
on similar have been rejected as Wont Fix. This is because as per product group Tethering for delegates is not a supported scenario.

Below are few of the BUG requests that were rejected for the same reason. And in all cases the issue is same.
1. Delegation Calls default to speaker phone
2. Delegation Calls default to speaker phone instead of the preferred device when pairing with Deskphone
3. Delegate toast does not contain "on behalf of" information when call is for Boss and LYNC phone is tethered
More Details:
The impersonation code used in boss/delegate scenarios is not available in Lync Phone Edition.
As a result, on-behalf-of features are not available from phone devices
when they are tethered via USB cable to a workstation.
When multiple devices are attached to a workstation, the Lync client will typically respect your preferred device when answering an inbound call.
However,
if you are tethered to a phone device using a USB cable, your deskphone pairing state is set to be TRUE - even if it is not the preferred device.
