How do you configure the blocking rule? You could try to use Domain Name Set to block Youtube.

Please check the TMG logging to see which Firewall Policy rule matches this traffic. Then create a rule to deny them and place this rule above the allow rule for HTTP/HTTPS.

I'm experiencing the same behaviour. Make a deny rule with a url set used to work for youtube.com, *.youtube.com. Now it isn't working any more.
