Trying to create new list in Service Manager Console 2012

I am not able to create a new list in Service Manager console 2012. I am trying to simply generate a new form with a couple of checkboxes so that I can use it for new tickets. 

Steps I followed:

1. Go into Service Manager Authoring Tool and create a new form.

2. Seal the management pack by creating key and exporting 

3. Import management pack- I can see it get imported.

4. List is not created from management pack being imported.

What am I doing wrong here?

For creating list on authoring tool, follow this steps,

1- Specify Data type of property as list.

After create related list item.

Configured property details must same as below.
