Hi,

Can you check that the System Center Data Access Service is running on the Management Server?

It seems that this management server is not able to communicate to the other server using TCP port 5724. Make sure this port is open on both Servers. There might be a Firewall in between or the Windows Firewall is blocking traffic.

Make sure your SQL servers that store SCOM database can be accessed. You may need to reset password for action account and data reader account, after that don't forget to change them in the corresponding service which logon as those account.
