I realise that this is strictly what you asked for but this is a dashboard that will display that data for you. You can add in a where clause on the performance data to only pull back free space less than 20%.

If you did want a report then in SCOM, I'd be tempted to an SLO for the collection rule and then run a report against the SLO.

Just remember that reports query the DW and not the database. They are also aggregated (hourly \ daily) and not the raw data.
