How can I create a virtual machine or migrate the converted vhds by disk2vhd to either Hyper-V or SCVMM? 

in effect while them (the disk) are .VHD you should carry them by any of this solutions. by the way, if you want to have the entire machine configuration, you should try a solution accelerator called Virtual Machine Converter that can handdle even VMWare machines to VHD.

I assume that you want to create a virtual machine using the converted vhd file .

The only thing you need to do is create a new VM in hyper-v host and you can select the existing VHD file during that wizard .

If that hyper-v host was added into VMM , after you create the new VM with existing VHD that VM will arise in the VM list in VMM console .
