There is article Custom Notification workflow on activity assignment or reassignment about sending notifications if Assigned to User changed. I'm trying to do the same not manually, but with Authoring Tool. But cannot find fields Affected User and Assigned to User in MP.

Is it possible or fully manual work?

It seems it's not possible with the Authoring Tool. So you have to code this manually in the XML code.
