Azure SQL DB Pricing calculation based on number of DBs in a single server or number of Servers only ?

The calculator

shows an Azure SQL DB (S0, United States, 744 hours, 1 database) costs $15.03\ month.

To be absolutely certain, this count is based on the number of databases on a single Azure SQL DB not the number of Azure SQL DB servers being used. Correct?

P.S. If my interpretation is correct, adding more databases to a single Azure SQL DB Server will increase the monthly charge.

There are two options single database and elastic database model which is in preview.

Yes, the price is per database.

Those options apply to Azure SQl DB. Correct?
