SCOM2012 R2 running on Windows server 2012 R2

We are continously receiving email alerts "Logical Disk Free Space in MB is Low".

When we look at the history of the alert, we see that it is activated and resolved simultaneosly by the system.

Can you help me to troubleshoot this.

This is because of frequent state changes, could you please open the object explorer and see how many times it has changed state.

Is this the monitor that's in the Server MPs or is this one you created yourself?
