1. You need a separate server. The HT and CAS roles cannot be clustered.
2. Any combination will work.
3. If you're using clustering, you need Enterprise edition. If not, you can use Standard. There are certain advantages to using SCR with a clustered mailbox server even if it's a cluster of one node because you can use
the RecoverCMS method to restore service.

Hello
In Exchange 2007 when you do the installation you will see active mailbox cluter and passive mailbox cluster you cannot install CAS and HUB with Cluster mailbox. You required Enterprise edition to have clustering
As Ed Crowley mentioned you need a seperate server for CAS and HUB which is used for File Share Witness between the active node and Passive node.
SCR is available in the Standard Edition of Exchange 2007 SP1. If a Mailbox server in an SCC or CCR environment is used as the SCR source, the Enterprise Edition of Exchange 2007 SP1 is required because Enterprise Edition is required when clustering
Exchange 2007. If a standby cluster is being used as the SCR target, the Enterprise Edition of Exchange 2007 SP1 is also required.
Thanks
