Nothing. However the attribute is synced in the other direction so it's not recommended to change it. A change to Exchange's primary proxy address will overwrite this value.
See proxy sync scenarios here:

Ok I tried to change email adr from E-mail field in AD and it changes also the primary email adr in AD. Is that normal?

Are you talking about the "E-mail" field on the General tab?
If so, this is the "mail" attribute and is not actually part of mail routing in Exchange 2007 or 2010, but is used for
some purposes in 2003. The best practice (especially for 2003) is to keep them identical.
If you have ESM installed on your machine, the ADUC GUI will try to keep the attributes in sync. But you can change the attribute directly (without ESM installed) and it won't update Exchange.
Why do you want to change the values? What's the bigger picture?

...but the fact is it updates Exchange 2003.
I'm just curious because some peoples told me that if I change it in E-mail field in AD from General tab, RUS will override it and put the original email back. So I tried it and if I change email from that field it also change primary smtp in E-mail
Adresses tab.

Hi SAMATA,
Exchange stores and uses many kinds of attributes about the e-mail addresses of a recipient, such as proxyAddresses, tagetaddress, mail etc. For detailed, you can
refer the article given by Mike.
The proxyAddresses attribute will be display when we open the properties of a recipient at the “E-mail Address” tab.
The mail attribute can be edited on the “General” page of the specific recipient in the Active Directory Users and Computers. And the value corresponds
to the primary SMTP proxy address.
So when we change mail attribute, the primary SMTP entry in proxyAddresses must also be updated so that both addresses are synchronized.
And in addition, I do the test indicate the RUS does not override the new e-mail address to the original e-mail address.
If you have any questions, please feel free to let me know.
Best regards,
Yang
