Perhaps your offline address book has not updated. Can you see if you also have duplicate listings in OWA? This does not use the offline address book and will help us rule it out. Mike Crowley Check out My Blog!

Are you able to see this in outlook non cache mode ?. If you are not able to see users duplicate entry in non cache mode then its problem of OAB as Mike said above. Do you also have any other complain from users like new mailbox's are not showing on OAB ?. I would suggest you to update OAB at your mailbox server and chek after some time by download OAB.
