Does the user have any delegate with any other mailboxes?
Can the user reproduce this issue via OWA?
Please use Cleanreminders switch to try getting rid of the reminder:
To do this, press Windows key + R to open the Run dialog. Type outlook /cleanreminders in the field and press OK. (Make sure you have a space between Outlook and the slash.)

not sure about OWA but will check.
The reimders - guy is using a MAC actually with OL 2011.

How many devices are syncing to the Calendar?
Just check if there are some mobile devices syncing, such as IPad, IPhone.
If so, please disable the sync with mobile devices to test the result.
In addition, Im confused which version of Outlook the user is using.
