This query was original posted in the Outlook forum here, although I was directed here. The issue seems to lie with the "Conversation History for Office 2013" add-in, although I not entirely sure. Please check out the other post for full details, although I have added brief comments below.
I started seeing this issue on a particular user and believe that there is something amiss not with the Outlook profile, but with the specific user account on a machine. Repairing/reinstalling Office does not fix the issue, and the same is true of creating
a new Outlook profile, yet a different user logging on the same machine but configuring Outlook and Lync to look at the affected users mailbox did not have the same issue. So it seemed to be that settings were persisting in an account that were not removed
by deleting the Outlook profile, and perhaps the root cause of the problem is the fact that the "Conversation History for Office 2013" (OcOffice.dll) add-in is not showing in the users profile.
It would appear that the registry keys relating to the FormRegions, and the add-in itself that gets added to Outlook occurs when Lync first starts (after a Outlook profile has been created, but a user doesn't even need to logon to Lync in order for this info to get added). The registry setting are for the FormRegions:

and the same is true of the "Conversation History for Office 2013" add-in registry info which points to OcOffice.dll.
However, the KB Article http://support.microsoft.com/en-us/kb/2000886, suggests to remove an re-add the "Conversation History for Office 2013" (see the "Cannot integrate with Outlook because Outlook mail profile is not configured correctly or cannot connect to Exchange Web Services - Method 2"). Problem is, it simply won't re-add to Outlook. I even tried this is the working account (i..e removing the Add-in, then re-adding) and I can't complete this in this profile either. Restarting Outlook/Lync makes no difference.
Has anyone got any other ideas?
