Columns in MS Word 2013

I am trying to create an "action" column to the right-hand side of a set of Minutes.  The column only needs to be a little skinny one for people's initials, however when I create a column the whole document runs on into the column - how can I create a space/column to the right-hand side which won't then run on with the entire rest of the text?  What is happening is all of the text of the entire document is running on into the column, but I want the text from the main document to remain separate from the "action" column I'm trying to create.  I know there must be an easy way to do this and have Googled it but as it's quite specific I'm not initially seeing anything helpful on tutorials.  Help much appreciated as always!

Could you provide the steps on how you created this column?

I'm not quite sure what you mean by "the whole document runs on into the column". If possible, please provide a screenshot here for reference.

Instead of creating columns in the Page Layout tab of the ribbon, I'd create a borderless two-column table, with a row for each part of the minutes.

The dotted lines in the screenshot below are table gridlines; they are normally displayed on the screen only, not in the printed output. You could add cell borders if you wish.
