Auto Attendant ARS finds name and repeats it back as two options

When a caller calls the auto attendant, they say the name and it finds them and then repeats the name back as two different options although they are the same. It does this for every person in the address list. 

It is doing this on exchange 2010 UM and exchange 2013 um. On 2010 I worked around it by running a task to run the galgrammargenerator.exe every day on the address list and that fixed the double names. Now on 2013 the galgrammargenerator is no longer there and done by another method. So now I need to figure out why it is repeating the names back twice because I can not work around it. 

Any help is appreciated.

From Exchange 2013 the GAL Speech grammar is stored in the Arbitration mailbox

Can you please try restarting the Microsoft Exchange Mailbox Assistants service and see the results

Also run Get-ArbitrationMailbox and see if everything is ok with the Arbitration Mailbox

I have restarted the service and the server with no change. 

Running Get-Mailbox -Arbitration gives me this.

Do you have a phoneticname set for the user having the duplicate problem?  You can check by powershell

get-user smtp@address.com | ft name, phonet*

Yes, we set phoneticnames for each user because our display names are Lastname, Firstname.

It seems to be related to the comma in the Display Name. Once I remove the comma from the display name the issue goes away. This is not ideal for Lastname, Firstname format.
