In exchange you have to configure the web services internal and external URL's properly in all of your exchange servers and also make sure it is accessible in web browsers from the internal network and external network as well.
For the OOF to work properly from external world , we need to make sure that the web services path has been allowed on the reverse proxy firewall if you have it on your network.Then check the OOF settings on the remote domain settings in exchange.

I checked the URL's for internal and external and it's accessible in web browsers.
I checked th OOF setting by running "get-remotedomain" and i get this:
is this correct?

if ews url ok set the following eventloglevel expert:
MSExchangeMailboxAssistants\Service
MSExchangeMailboxAssistants\OOF Assistant
MSExchangeMailboxAssistants\OOF Library
turn off oof. and turn on again. send email to user and check eventlogs.
if published outlook anywhere to interent check oof with https://testconnectivity.microsoft.com/
Does it work internally or failing both internal and external?
Does it fail when sent directly to user or thru Distributionlist?
By default OOF thru Distributionlist is disabled.
If thru distribution group,run this command in EMC:
Set-DistributionGroup -Identity "DL.Group.Name" -SendOofMessageToOriginatorEnabled:$True

Our OOF is failing both internal and external. Fail when sent to user.
your connectivity error above failed on autodiscover.
Is it correctly setup in internal and External DNS?
Open cmd and run the following:
nslookup autodiscover.domain.com 8.8.8.8
If on internal network run the following:
nslookup autodiscover.domain.com

Please check the autodiscover service in your client side and server side.
In client side, please run Test E-mail AutoConfiguration tool to check whether the autodiscover service is connected siccessfully:
Open Outlook - press CTRL key - right click on the Outlook icon from right bottom corner taskbar - Test Email AutoConfiguration. Put your email address - uncheck use guessmart and secure guessmart authentication - click Test to check your Autodiscover service.
Please collect the Log tab information here for further analysis.
In server side, please run:
Get-ClientAccessServer | FL Identity,AutodiscoverServiceInternalUri
