404 Error After Successfully Logging in to OWA/ECP in Exchange 2013

Hello,

My apologies first if this would be better asked in another forum but this one seems to fit best.  I am running Exchange 2013 on Windows Server 2008 R2 Standard and am experiencing an issue after logging in to ECP and OWA.  After entering my credentials (all credentials experience the same issue) I am redirected to a 404 error for both \ecp  and \owa.  The odd thing is that Re-entering the url completes the login.  How do I fix the appearent lack of the login.aspx?  If my description isn't sufficient please let me know.

Make sure authentication is set to Basic not Form Based on OWA and ECP virtual directory in IIS.
Disable Form Based authentication and restart IIS, check the issue

What if you want to use FBA?  We are using FBA and on first logon on any computer the 404 page is displayed.  You can click the "Refresh the page" button and it takes you into OWA.  When you log out and close IE, the next time you logon on that same computer it takes you right in.  If you clear your temp files (cookies etc. included), you get the same 404 page.  I've attached an image below-

It is my understanding that for coexistence with exchange 2007 we have to have FBA enabled on both ends.

I know that once we de-commission our 2007 environment I can change to basic, but that won't happen overnight and I don't want my users to have to re-load on login to owa (it looks bad)

Did you ever get this resolved. I am having the same issue and I have tried everything on the net. Everything rebuilt all folders reset permissions. I have a fresh 2013 install and this issue will not go away.
