I am trying to print a form in SharePoint 2010.  I have added a print button which gives me the windows print dialog.  I select all pages (there should be 2 pages) and print.  The first page prints.  But, the second page has the header (form title) and footer (web address) but nothing else.  This is what happens in Firefox.  With IE, only one page is printed - which is half the form.

I am at a loss as to why this is happening.  Any ideas where to start?  There was some indication it could have to do with RTF text boxes - however; all text boxes are plain text.
