Does your FIM Service Service Account has a mailbox of it's own?http://setspn.blogspot.com

Every time I got that error it was when the service account that FIM uses to send notifications was sending emails to a non-existent or invalid email address. This happened when email addresses weren't being populated in the Portal which was fixed. Later on it recurred when email addresses were no longer valid in AD but the NULL value was not backflowed into the FIM Portal.

Can the FIM Service send e-mail? That stack trace looks like the FIM Service cannot connect to the Exchange Server. Remember you defined an Exchange CAS during installation, and probably said to use HTTPS and enable polling (EWS) unless you don't have exchange in which case you just specified an SMTP server. That error can be thrown by no permissions (unlikely in the case of a CAS server) and also by no response at all IIRC.
