Please assist me. How can I add additional field to approving form? I hav : request list; tasks list; approve workflow. I need to add several fields to approval form tasks list.

I can open form from disigner via infopath.

If you want to customize task form of Approval Workflow, I'd recommend you using SharePoint workflow based on 2010 platform.

Since task form associated with task action in 2013 workflow, it is not easy to customize. However, if we are using 2010 workflow, we can see the form directly in SharePoint designer, then we can open it with InfoPath form to customize:
