Use separate login page for Forms Based and Windows Authentication
We have successfully implemented a SharePoint 2013 farm and web application using FBA.   We used the FBA Pack which sets up a default page which asked the users to select Windows or Forms Based authentication.  We would like to create two login pages, one for Windows and the other for FBA.   We are also using Host-named Site Collections.   What would be the best method to accomplish this?   We have tried to customize the login page, but it still requires us to use both Windows and Forms Based on the

According to your description, my understanding is that you want to create custom login pages for both FBA authentication and Windows authentication.

I suggest you can create a custom login page with a dropdown list to select which authentication type to pass the site collection.
