Is it possible to initiate a survey with a workflow?
I want people to fill in a survey whenever they upload a file to a certain library.

Hi,

Per my understanding, you might want users fill out a survey when uploading a file to library.

If this is the case, then there will be no need to use a workflow.

A possible solution using the OOTB feature is that you can add the columns needed into that library, then when users upload file to the library, the EditForm.aspx page of this library will pop up with the custom columns you added. You can then view the user input in the DispForm.aspx page of the same library using the View Properties feature:
