SharePoint form to be completed by specific person

I want to create a form (of various ratings scales and text boxes) which I want to then allocate/assign to specified users within the organisation.

I am able to design the form by creating the various lists, but do not know if it is then possible to allocate/assign it to someone for completion.

Scenario:

I complete a project and require feedback on my performance. I want to be able to allocate the pre-designed form (survey) to the head of the department for which I completed my work. I want this to be driven by SharePoint, i.e. I must be able to complete certain generic information on the form (name, date, department name, etc.), then select the name of the person within my organisation and then save the form. I want the save function to then notify the assigned person (via email) that he/she should click on a link and complete the form.

Is this possible?

I am using SharePoint 2010 as well as infopath.
