OSD failed with following error.

OSD Task sequence keeps failing with following error. Hard drive was replaced on this defective unit and it keeps failing while trying to rebuild this system. following are smts logs.

Please look at the execmgr.log file. That should have the lines for when it tried to install your app.

My guess is that the content failed to download. It looks like you are using Nomad - you may have to look at the Nomad logs to see if there are any download failures. For downloads done by the SCCM client, you need to look at CAS.log, ContentTransferManager.log and DataTransferService.log.
