Issue:

I have a Nintex Live form and when submitting, it is not creating List items in SharePoint 2013.

Solution:

After reviewing the heavy JS customisation, a comment from one of the server guys helped solve the problem.  

The form was developed by an external vendor and recent AD account clean up activities had the account (that deployed the form) disabled.   

I re-published an unmodified version of the form and it worked as expected.
