We have a requirement to build SCCM 2012 in one of the Datacenter - A after complete rollouts etc and migrate it to Datacenter - B once the DataCenter - A is completely shuts down.

The Datacenter A is similar replica of Datacenter B , machines are accessible to both DC.

Can someone help on best practices, on what changes will it take place if IPAddress of SCCM server changes, what re-works will be required on SCCM client part, what measures and best practice have to be followed. any AD schema changes, DNS changes etc.

The change in IP shouldn't be a problem as long as DNS is updated properly. Just keep in mind that you can't move the site server to a different domain.

I also come across DC movement situation, and also looking for best practices and approach of SCCM 2012 data center movement. Can someone help to share their experience?

It's no problem as long as the server name and domain membership remain unchanged. See above.
