You can install a primary site in azure. You can also create CAS in Azure so i don't see why you would not be able to have your Azure server join your existing CAS
Set up a single stand-alone Primary site in the Microsoft Azure Virtual Machines environment to manage Microsoft Azure Virtual
Machines that are running Windows Server or Linux in the same virtual network.
Note The all-in-one, stand-alone Primary site is a single Microsoft Azure Virtual Machine that runs all required

But it is not supported, the only supported scenarios as the blog post above points out as well, are
- manage you azure VM's from an On-prem SCCM infrastructure
- install an all in one SCCM Primary site with the necessary roles and only manage the servers in Azure IAAS with it.
That is the only supoorted scenarios.

SCCM is now fully supported in Azure. Including CAS etc.
And yes, you can install servers in different regions (Azure Sites).

The question acutally is:
Is it supported to install a Primary Site in Cloud and attach it to an on-premises CAS?
So the CAS is on-premises.
Well if they say SCCM is now fully supported and and other primary site you create can join a CAS on azure i don't see why they could not join a CAS on premise.
Maybe some of the expert here could better answer your question (maybe some people already tried it).

The new Azure support for Hierarchies and your scenario will be supported in Configuration Manager vNext, it is stated in the blog post for the Technical Preview: http://blogs.technet.com/b/configmgrteam/archive/2015/05/04/announcing-support-for-windows-10-management-with-system-center-configuration-manager.aspx
"Support for installing Configuration Manager on Azure Virtual Machines Similar to how you can install ConfigMgr on Hyper-V today, you can now run ConfigMgr in Azure VMs. This provides flexibility to move some or all of your datacenter server workloads to the cloud with Azure."

So as far as I know your scenario is not supported with SCCM 2012 but is in the roadmap for SCCM Vnext.

Just being curious: what kind of traffic to you want to minimize with that scenario?Can I install a Primary in the cloud and connect it to current hierarchy to minimize traffic across the connection?
Example: OS Image replication if I chose to create the images for Cloud on a cloud Machine itself. But lets set that aside and talk about a normal cloud plus on-premises scenario...Is installing a cloud DP enough to manage things like Software dist, inventory, patch management etc?
Is cloud dp installed on one of the Azure machines?
