I'd like to custom the initial form of a sharepoint workflow. I only need to add some agreed text and a radio option with yes or not.

I'm using SharePoint Foundation, so I can't use infopath. I download the aspx file from designer, and change the text, but when I upload it, the text not appears when I start workflow.

How can I add any custom text, to the initial form in a sharepoint workflow?
