SharePoint 2013 workflow in SharePoint Designer

When you are using the "Create List Item" Action you click on "this list" and a dialog box pops up. You select the list the workflow with create the item for. Then you have to set the Path and Name (*). if you try to save without putting this in you get a message letting you know this is required. What Path and Name is it looking for?

I am doing this on a document library if that matters at all.
