We have SCCM 2012 and WSUS in the different server, SCCM server can't accesse Internet, WSUS can download update from Microsoft and already downloaded many updates in server, I have some questions when I create ADR.

1. In the step of Deployment Package, I need create a new deployment package, I don't know how can I choose package source, create a new path in SCCM server or use WSUS path?

2. The Download Location, which option can I use?

1. I would use a new package for my ADR and point it to a empty folder where it can download the updates to.

2. Is the WSUS configured as a SUP in Configuration Manager or is it a standalone Upstream WSUS you are using?

If you want to be able to use ADR the Site Server will download the updates from the Internet so you either need to grant that server access to internet or use an upstream WSUS server that has access to internet and is configured to sync updates and then SCCM can use that one instead.
So it depends on your setup.

Because my site server doesn't have Internet access, I have to use my WSUS server.

You are correct. You need to choose "Download software updates from a location on my network", and the network path should be the WSUS content share.

I added WSUS content share to "Download software updates from a location on my network", but when I run ADR, I received error from ruleengine.log, the error is "Failed to download the update from UNC content source. Error = 2"

Error = 2 means "File not found". 

In the blog I posted above, "Configure your upstream WSUS server to download all the updates from the products and classifications you the selected during the WSUS configuration and cache them locally. Configure the above setting also in the WSUS console of your SUP. Normally its not recommended to set WSUS configuration settings on a SUP directly in WSUS. But this setting is not managed by SCCM."

Please make sure the WSUS server has been synced with Internet.

I used WSUS console to configured WSUS server donwload all update to local, WSUS can sync with Internet, but the error still exist, the ADR had an error said "The file you specified could not be found. This may be because it is not signed."

Have you tried to re-sync all the updates from SCCM console and monitor the wsyncmgr.log? Please create ADR after the sync completes without error.
